And I understand that JackRabbit LLC will invoice me for the delivery fees and Tip as collected from EZCater based on the delivery fee schedule. This ACH Authorization Form authorizes JackRabbit, LLC to initiate debit and credit entries to the bank account identified above for payment of goods and/or services. This authorization remains in effect until revoked in writing by the account holder.
